Applicant’s documents
Send the following:

a colour photocopy of both sides of your Hong Kong permanent identity card
full colour copy (every page including blank pages) of any current or expired passports from other countries that haven’t been cancelled
And 1 of the following from the last 12 months, that shows your name and address and/or residency:

letter from your employer
medical card
letter from your local council or a government department
voter’s card
visa or residence permit (a colour photocopy will do)
education record for example a school report
immigration documents
And 1 of the following that has your photo on it:

non-British passport that hasn’t expired
national identity card (a colour photocopy will do)
driving licence
any government or local government produced document

如果我已改名，但係改名契得中文版，可以點申請續期BNO? 我問過律師樓，佢話唔會俾到英文版改名契我，請問登記事項證明書/到民政事務署宣誓可否代替改名契作為證明？ 如可，宣誓內容係咪簡單講我由幾時將原用名改為現用名就得？

我想續好耐，但真係因爲依個問題唔知點續。如有資訊，請幫忙，感激不盡

出世日期個D唔知 無打
係個box度有解釋下點解會唔知e.g 過左身太耐

1. 本人出世紙-正本
2. Old bno passport
3. Address proof (圖書館預約圖書）-正本
4. 爸媽結婚證明-正本
5. 本人特區護照-color copy
6. 本人+爸媽身份證-底面color copy
7.副簽paper form
8.副簽人既bno 有個人資料個頁 既color copy

以上用香港郵政speedpost 71hkd左右
1星期唔駛運到HMPO office
郵局職員非常熟手 應該係呢幾個月太多人幫襯
前日已收新BNO 同埋所有寄出文件

我問左了 佢叫我聽日call佢 叫佢幫停止我個遺失申請
我覺得好麻煩，定係我扮唔見左算

我都有 好多人都有
不過暫時好似未有人要去

如果要停cancel, 我都會扮唔見算，不過而家要cancel都好多人要打電話比佢地。
