All I can say is be honest with the Home Office. If you wrote it down in the application form, then they cannot come back and say that you did not tell us the truth.

According to the rules in the above link, the Home Office can refuse people an UK visa for 'failed to pay a charge or charges with a total value of at least £500 in accordance with the relevant NHS regulations on charges to overseas visitors'. (See A320 section 22)
