如果你的副署人為英國國民或愛爾蘭公民以外的英聯邦國民、歐盟國家國民或美國護照持有人，副署人必須提供副署人護照個人資料頁的彩色影印本。

How to confirm someone’s identity
You need to:
• check the applicant’s details are correct
• check the photo is a true likeness of the applicant
• provide your details and say how you know the applicant
• sign the declaration
Give the applicant:
• the completed form – don’t fold it
• a photocopy of your passport’s photo page – unless you’re a UK or Irish
passport holder
Do this as soon as possible. We can’t work on the application until you’ve
confirmed the applicant’s identity.
