我諗基本上你去門診睇醫生同佢講斷咗字韌帶,叫佢寫轉介信去物理治療部就得,不過佢排期係分priority,未做手術前要排幾耐期就唔肯定了。我嗰陣都驚要排幾個月,差啲疊埋心水去私家做。

物理治療師認真答

首先想比你知道嘅野係物理治療同西醫都係基於research嘅醫學方式 下邊講嘅所有野都可以比番Reference你

通常對於係運動中間整斷前十字韌帶(ACL)嘅Case 第一樣要佢地考慮嘅野係做唔做手術
上邊有啲巴打講如果你踢衛生波可以唔做係True
以往見到有好多人如果主要玩linear sports eg跑步單車GYM 而唔係以籃球/足球等等嘅dynamic sports做主項 其實係可以唔做
不過近5-10年嘅research可以見到唔做ACL reconstruction (ACLR/前十字韌帶重建手術) 其實都有好大機會可以回復正常水平 但實際你個人可唔可以就需要做一連串嘅Test 例如你嘅跳躍能力去判斷

咁假設你已經決定做ACLR手術 咁就可以Go thru個流程

首先第一個問題:手術前需唔需要做Physio?
Research上係要 我地可以見到across唔同專項嘅運動員 如果手術前有做Physio 好番嘅速度/可以return to sports嘅程度/再整斷韌帶嘅機會都會大大減低
手術前嘅目標主要係減少腫/確認你隻腳可以伸直到同另一隻腳一樣/盡早開始訓練多啲肌肉/協調能力

呢到有少少題外話 如果你聽呢到啲巴打講出去搵Physio淨係以有無幫你做好多機做指標 咁大完全錯左方向 機器嘅用途只係very weak evidence for slightly幫助你減少痛楚/腫 你要理解嘅係ACLR嘅rehab period係最少九個月先可以重新做你嘅專項運動 而呢九個月應該只係得非常非常非常非常少嘅時間係做緊機器/passive treatment 相反應該主力係以運動黎強化番隻腳

第二就係手術嘅程序 不過呢個理應係要同醫生夾 唔講太多都唔影響個大方向

第三係手術後嘅處理 亦都係最多人有問題嘅地方
通常做完手術我地就想即刻見到啱啱做完手術嘅Patient 因為我地唔係太想你rest得太多 都唔想你完全唔理隻腳覺得佢會自己好

通常手術後嘅傷口唔大 但都要一至兩個星期at least去癒合 而入邊嘅軟組織就要可能6-8個星期 所以最頭嘅一兩個月都係會比較腫 亦都非常需要你個人去幫手自己係屋企處理個腫
其次就係幅度盡快可以伸直
再黎就係對四頭肌/其他下肢肌肉嘅控制同感受度
呢三個目標最好就係兩至三個星期完成 之後就係慢慢再加入下肢其他野嘅訓練

整體黎講 成件事係真係好長時間 最想你要得到嘅take away係千萬唔可以rely on passive treatment 逐步強化隻腳/另一隻腳係必須

Physio愈早開始愈好 手術前你行得走得主要係教你management同埋自己係屋企/gym可以做嘅訓練 理應唔洗好密咁去

公立一定唔會教得晒你所有野 可以睇但建議seek 2nd opinion

It really depends on your age, your expectation in future sports and physical activities and your determination in making the knee back to normal. Normally, people age under 25, surgical repair is recommended but not a must.

Choose BPTB (Bone patella tendon bone) graft method if you want to re-engage in more vigorous sports after surgery. Hamstring method is for general repair as the laxity of graft is more which gives more translational movement of the knee during start-stop manoeuvers. ACL normally gives much better surgical outcomes than PCL. Consult your orthopaedic surgeon for the need and outcomes of a PCL reconstruction.

Unless you are in elite sports clubs, normally, they will do surgery before the knee develops swellings and muscle wasting, i.e. surgery within 1-2 days to minimal reciprocal inhibition of quadriceps wasting. In your case, be quick to settle inflammatory signs, redness, swelling, pain and increased in temperature over the knee. Do lots of close chain quadriceps and hamstring exercise, such as, leg press etc. Avoid open chain exercise, such as, dead weights kicking. Proportion of Quadriceps vs Hamstring strengthening is approximately 2:1.

If you don't attempt surgery, you have to stay active in gym, strengthen quadriceps and hamstrings to avoid imbalance of the muscle groups. If imbalance happens, shearing forces to soft tissues inside the knee joint will cause early joint degeneration. Also, avoid sudden and jerky sports as your control over the knee is lessened due to the loss of both cruciate ligaments. Conservative treatment is totally fine for people which will not be active in contact sports, sports involving heavily in cutting and jumping.

多口問句依家上落樓梯,轉身果啲有冇力?係咪已經諗住一定會做手術?

係嘅話睇完物理治療記得自己努力練,因為手術只係俾左個foundation你。唔好好練返啲肌肉嘅話真係唔排除斷多次

個人感覺其實手術帶黎嘅創傷仲大,大腿肌肉萎縮同埋後腿抽左截筋後容易抽筋,provided that本身斷左後我膝頭都唔係太鬆,行得走得,踢下衞生波都可以

講到尾真係覺得做定唔做要諗過下先
