1）1997年7月1日前未滿11歲
2）身份證首次簽發日期為（06-97）或之前
3）香港出世而且出世紙寫有”Established”

Included child 係sar唔到！一係再sar父母問佢哋本bno有邊本有你名。一係直情報lost，等有ref no真係confirm cancelled passport 後，就課金申請。
