IB is much more rigorous than DSE and approaches university year 1 level. For example English includes literary criticism. Math includes M1/M2 content at standard level. So considering equlviance of academic ability it would make sense that local universities have lenient requirements for IB scores.

HKU MBBS admitts 36/42 for DSE and 44/45 for IB. I would say the IB requirement is more difficult.
You can't ignore that the students taking IB in the first place are already top ~20% in Hong Kong. Of course the percentage of them getting high grades would be easier.
Even if I assume IB 7 = DSE 5* the IB cutoff (43/45) is only slightly easier.
